Transition Project

Project YES!

Project YES! (Youth Employment Solutions) is a systems change project that aims to increase competitive employment of Nebraska's Social Security Income (SSI), Social Security Disability Insurance (SSDI) and Childhood Disability Benefits (CBD) eligible youth in transition. The project is funded by the Nebraska Health and Human Services System through Nebraska's Medicaid Infrastructure Grant.

Through systems change, transition stakeholders will:

  1. Promote competitive employment opportunities for Nebraska's SSI, SSDI and CDB eligible youth with disabilities.
  2. Understand how Nebraska's SSI, SSDI and CDB eligible youth with disabilities can continue to receive Medicaid benefits and personal assistance services once employed.
  3. Understand how Nebraska's SSI, SSDI and CDB eligible youth with disabilities can use employment support programs to access employment.
  4. Work collaboratively to help Nebraska's SSI, SSDI and CDB eligible youth with disabilities to overcome other barriers to employment.

Project YES! is recruiting local community teams within the cities of North Platte, Kearney, Grand Island, Hastings, York and Lincoln to help increase the competitive employment of SSI, SSDI and CDB eligible youth ages 18 through 24. Together with Project YES!, local community teams will:

Youth Transition

As adolescents become adults, they must assume responsibility for their health care. Adolescents with special health care needs face difficult transitions to adult care, and to achieving greater financial independence. As all children become young adults, health insurance coverage becomes a major factor in their access to health care, because it is during this time that they are in personal transition between their parents and independence. On this site, you will find information, workbooks, links and other materials related to health transition issues.
